چکیده

New treatments for diabetes are always exciting in the beginning but as experience grows their position in management becomes less certain. This review focuses on DPP4 inhibitors and their place in management of type 2 diabetes. Although hypoglycaemia is rarely an issue, the effectiveness in reduction in blood sugar is greatest at the higher blood sugar levels, hence much of the value of using DPP4 inhibitors rests in the target HbA1c chosen for the individual patient. *Corresponding author: Gerald H Tomkin, Department of Diabetes and Endocrinology, Beacon Hospital and Department of Medicine, Trinity College Dublin, Ireland, Tel. +00 3531 2390658; Fax : +00 3531 2721395; E-mail: [email protected] Received December 17, 2013; Accepted January 24, 2014; Published January 31, 2014 Citation: Tomkin GH (2014) Treatments for Type 2 Diabetes; a Focus on DPP4 Inhibitors. Pharmacology, physiology, and mechanisms of action of dipeptidyl peptidase-4 inhibitors.

Dipeptidyl peptidase-4 (DPP4) is a widely expressed enzyme transducing actions through an anchored transmembrane molecule and a soluble circulating protein. Both membrane-associated and soluble DPP4 exert catalytic activity, cleaving proteins containing a position 2 alanine or proline. DPP4-mediated enzymatic cleavage alternatively inactivates peptides or generates new bioactive moieties that m...

Potency, selectivity and prolonged binding of saxagliptin to DPP4: maintenance of DPP4 inhibition by saxagliptin in vitro and ex vivo when compared to a rapidly-dissociating DPP4 inhibitor

BACKGROUND Dipeptidylpeptidase 4 (DPP4) inhibitors have clinical benefit in patients with type 2 diabetes mellitus by increasing levels of glucose-lowering incretin hormones, such as glucagon-like peptide -1 (GLP-1), a peptide with a short half life that is secreted for approximately 1 hour following a meal. Since drugs with prolonged binding to their target have been shown to maximize pharmaco...

Dipeptidyl Peptidase 4 Inhibitors and the Risk of Cardiovascular Disease in Patients with Type 2 Diabetes: A Tale of Three Studies

Dipeptidyl peptidase 4 (DPP4) inhibitors have been touted as promising antihyperglycemic agents due to their beneficial effects on glycemia without inducing hypoglycemia or body weight gain and their good tolerability. Beyond their glucose-lowering effects, numerous clinical trials and experimental studies have suggested that DPP4 inhibitors may exert cardioprotective effects through their plei...

Dipeptidyl-peptidase 4 Inhibition: Linking Metabolic Control to Cardiovascular Protection

Dipeptidyl peptidases 4 (DPP4) inhibitors are a new class of oral anti-hyperglycemic drugs for the treatment of type 2 diabetes (T2DM). They are also called "incretins" because they act by inhibiting the degradation of endogenous incretin hormones, in particular GLP-1, that mediates their main metabolic effects. DPP4 is an ubiquitous protease that regulates not only glucose and lipid metabolism...
